Comparison of the Effectiveness of Cognitive-Behavioral Hypnotherapy and Acceptance and Commitment Therapy on Pain Catastrophizing and Uncertainty Intolerance in Fibromyalgia Patients

Abstract
Background and objective: The aim of this study was to compare the effectiveness of cognitive-behavioral hypnotherapy and acceptance and commitment therapy on pain catastrophizing and intolerance of uncertainty in patients with fibromyalgia.
Methods and Materials: This semi-experimental study was conducted using a pretest-posttest design with a control group. The study population included all women diagnosed with fibromyalgia who visited medical and physiotherapy centers in District 1 of Tehran from June to December 2023. A total of 45 participants were selected through convenience sampling and randomly assigned to two intervention groups and one control group. The intervention groups received eight sessions of cognitive-behavioral hypnotherapy and acceptance and commitment therapy. Data were collected using the Pain Coping Strategies Questionnaire (Rosenstiel & Keefe, 1983) and the Intolerance of Uncertainty Scale (Friston et al. 1994), and analyzed through repeated measures tests and Bonferroni post hoc analysis.
Results: The effects of time, group, and the time-group interaction on pain catastrophizing and intolerance of uncertainty were statistically significant (P< 0.0001). The mean scores of pain catastrophizing and intolerance of uncertainty in the intervention groups were significantly lower than in the control group (P< 0.0001). A significant difference was found between the intervention groups from pretest to follow-up (P< 0.0001), while no significant differences were observed between posttest and follow-up scores for pain catastrophizing (p=0.066) and intolerance of uncertainty (p=0.068).
Conclusion: Given the findings, cognitive-behavioral hypnotherapy and acceptance and commitment therapy can be applied as effective psychological interventions for fibromyalgia patients in mental health and counseling centers.
